Procedures For Obtaining Services:

At your initial contact with UCCS you will be asked to complete an application for services and to provide a copy of your insurance.  You will then be scheduled an assessment appointment.  At that appointment you will meet with a counselor to discuss why you are seeking services and to explore what services UCCS can offer that might be helpful in accomplishing your goals.  If you agree to the recommended treatments, you will then meet with an orientation specialist to sign treatment agreement forms, discuss fees, and review client rights and program rules.  You may meet with a counselor to finalize your treatment plan on the assessment day or you may do this at the first appointment with your assigned staff person.  Once your treatment plan is established, you and your counselor will review it about every 90 days to make sure you are making progress toward your service goals.

Consumers who have Medicaid or other another type of government payer source or who wish to be considered for a sliding fee scale are required to provide proof of income and household size.
